  • Where applicable, we align our privacy and security practices with HIPAA requirements - U.S. HIPAA (Health Insurance Portability and Accountability Act)

 

Your information is kept confidential and protected, subject only to your consent or lawful disclosure requirements. Second Origin acts as a Health Information Custodian under Ontario’s PHIPA.

Before starting services, you will sign a Service Agreement & Consent Form that explains what information will be collected, how it will be used, and your rights.

  • Request corrections or updates to your records

How We Share Information

 

We will only share your information:

Record Retention

 

Records are kept for at least 10 years after the end of services for an adult or 10 years after a youth or child has turned 18, or longer if required by law/licensing boards.

 

After this period, records are securely destroyed (shredded or permanently deleted).

Records can also be requested by current clients, or legal guardians at any time.

 

 

Communication & Security

 

Texts, phone calls, and emails are intended only for you.

We use your phone number to send SMS messages related to inquiries, scheduling, appointment confirmation, or service coordination.

 

We use secure platforms, but no system is 100% risk-free.
